Medical History Management

Patient medical history is thoroughly documented in Vehrdict EHR. From initial intake to ongoing updates during each visit, all relevant information such as previous diagnoses, surgeries, allergies, and family history is securely stored. VEHRDICT integrates voice dictation and AI transcription, ensuring that all history records are immediately available within the patient’s chart, with any updates added in real-time. The medical history timeline is clearly visualized, making it easy for providers to review and update critical information efficiently.

Adding Medical History

Start from the Patient Dashboard:

• Navigate to the ‘’‘Patient Dashboard’’’ for the selected patient. • In the Patient History Section, Click on the ‘’‘Edit’’’ button to create a new entry for the patient’s medical history.

Using AI Voice Input:

• Click the microphone icon at the top of any page. • Say 'create medical history' and then speak the medical history details. VEHRDICT AI will automatically transcribe and add the information to the patient’s chart.

Viewing and Managing Medical History

Medical History Timeline

• The medical history timeline provides a clear, chronological view of all entries.

• Use the timeline to quickly reference past diagnoses, procedures, or any changes over time.

• All new updates appear instantly in the timeline, ensuring a comprehensive view at any point.

Editing and Updating History

• Navigate to the patient’s Medical History tab, and select any entry you wish to update or edit.

• Use the menu options in the top right corner to add new entries or modify existing ones.

• Click the microphone icon for AI-powered updates or use the keyboard for manual entries.

Comprehensive Overview

• Each patient’s history is integrated with other records, such as lab results and prescriptions, ensuring a complete view.

• VEHRDICT’s AI suggests relevant updates or flags inconsistencies for review, ensuring that each patient’s history remains accurate and thorough.
